Patients with a new cancer diagnosis may wish to preserve their fertility. This document explains the protocols for access to public-funded fertility services provided by North Metro for WACHS.

Email referrals to KEMH.ReproductiveMedicine@health.wa.gov.au including the patient’s name, date of birth, medicare details. Provide details of relevant medical and oncology history and recent STI screening results.

For females 25-34 years, include a CST from the past 5 years. Females over 35 are ineligible for referral.

Only patients referred to the Contractor by the Reproductive Medicine Services team at KEMH are eligible for public funding.
